Across all posited aetiologies, contact granulomas are more commonly observed in male patients than in female patients, and more commonly in adult patients than in pediatric patients. [7] In cases where gastroesophageal reflux disease is identified as the most likely cause of the granuloma, the patient is most commonly an adult man in his 30s or 40s. [7] When contact granulomas do occur in female and pediatric patients, they usually occur post-intubation. [7] There are no observable age or gender trends within the category of post-intubation patients suffering from contact granulomas, nor within the causation category of vocal abuse history. [7]

Aspiration pneumonia is typically caused by aspiration of bacteria from the oral cavity into the lungs, and does not result in the formation of granulomas. However, granulomas may form when food particles or other particulate substances like pill fragments are aspirated into the lungs. Patients typically aspirate food because they have esophageal, gastric or neurologic problems. Intake of drugs that depress neurologic function may also lead to aspiration. The resultant granulomas are typically found around the airways (bronchioles) and are often accompanied by foreign-body-type multinucleated giant cells, acute inflammation or organizing pneumonia. The finding of food particles in lung biopsies is diagnostic. [12]

How is it treated?
Vocal cord granulomas may resolve with voice rest and voice therapy to reduce muscle tension and repeated phonotrauma, reducing silent reflux with medical therapy, and often adding topical treatment with inhalation steroid therapy.  Some granulomas do not resolve with conservative measures or are quite large, which necessitate surgical removal.  At the Voice Clinic of Indiana, surgery can offered in either a clinic setting (under local anesthesia) or a traditional operating room setting (under general anesthesia).  Occasionally, steroid or botulinum toxin is used as an adjunct to help treat vocal cord granulomas.
